New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All our new patients are advised to attend for a health check with our practice nurse. We would be grateful if this could be done as soon as possible in order to obtain more information about your past history and medication. You will need to bring a urine sample and will have your height, weight and blood pressure checked.
Medical treatment is available from the date of registration. Please contact reception for further information.
Proof of Identity
As part of the registration process, we need to see sight of original documentation proving your address and identity. Examples of Identity documentation include:
- Passport
- Driver's Licence
- Birth Certificate
- Marriage Certificate
- Medical Card
- National Insurance Card
Examples of Address documentation include:
- Utility Bill
- Bank Statement
- Tenancy Agreement
- Inland Revenue/Benefits Agency correspondence
Update of Patient Information
It is important that the practice is informed of any changes of name, address or telephone number. Please check at reception that your telephone number is entered on your records. Please notify us of your mobile phone number as well as your home number.
